Job summary

Compassus is seeking a Home Health Occupational Therapist to provide skilled OT services in a home health setting on a PRN basis. The role emphasizes compassionate, ethical care, team collaboration, and adherence to the Compassus philosophy.

The OT will develop and execute plans of care, perform assessments, and educate patients and families. Responsibilities include completing OASIS assessments, documenting thoroughly, supervising aides, and contributing to quality improvement.

Qualifications

• Bachelor degree in Occupational Therapy required.
• Active OT license in state(s) of employment required.
• At least 1 year of experience as an OT in acute care, home health, or rehab setting preferred.

Responsibilities

• Develop OT plans of care for patients.
• Visit patients for assessment and evaluation; communicate with physicians as needed.
• Complete OASIS assessments and implement plans of care.
• Provide education to team members, patients, and families.
• Evaluate outcomes of care and discharge planning when appropriate.
• Supervise paraprofessionals as needed.
• Ensure timely documentation and accurate EMR entries.
• Deliver skilled restorative treatment to improve joint/muscle function, pain management, and ADL training.
• Collaborate with interdisciplinary team to coordinate patient care.
• Uphold patient rights and safety, adhere to infection control procedures.
• Participate in quality improvement programs and policy development.
