Job summary

Banner Health seeks a part-time Home Health Occupational Therapist to evaluate patients, plan and administer therapy, and coordinate with the care team. You will deliver patient-centered interventions in home care settings across assigned territories, ensuring safety and functional progress.

The role requires an OT degree, current state license, and BLS certification, with strong documentation and communication skills. Flexible scheduling is available to support patient care across shifts.

Qualifications

• Completed OT program and eligible for OT licensure in state of practice.
• Current Arizona OT license and BLS certification required.
• Proficient with computer software used in patient care and documentation.

Responsibilities

• Evaluate patients and develop/adjust treatment plans.
• Provide occupational therapy interventions per physician orders.
• Document progress and communicate with care team.
• Educate patients and families; participate in discharge planning.
• Coordinate with COTA and support staff to ensure safe, effective care.
