Job summary

Alternate Solutions Health Network is seeking an Occupational Therapist to deliver home-based therapy across Cedarville, Jamestown, Wilberforce and Xenia, working Monday–Friday with no on-call or holiday rotation. You’ll be part of an interdisciplinary team, educate families, and ensure safe, evidence-based care in patients’ homes.

You’ll manage planning of care, perform functional evaluations, and monitor patient progress while adhering to Medicare guidelines and agency policies.

Qualifications

• Occupational Therapist licensed in the state of employment.
• Minimum one year of OT experience in an acute care setting; home care experience preferred.
• Valid driver’s license and auto insurance in your name as a driver.

Responsibilities

• Plan and provide skilled occupational therapy per physician orders; document observations and progress.
• Evaluate patient functioning and adjust OT plan of care as needs change.
• Improve range of motion, strength, and coordination through therapeutic activities.
• Minimize hospital readmissions by teaching preventative care and self-management.
• Educate and advocate for patients and families on disease, injuries, and therapy use of equipment.
• Supervise and coordinate care with COTA; communicate supervision results.
• Maintain compliance with agency policies, Medicare guidelines, and documentation.
