Job summary

Qualifications

• Must have a valid and unrestricted OT license in state of practice.
• Graduate of ACOTE-accredited program.
• If foreign-trained, credentials deemed equivalent by national evaluator.

Responsibilities

• Assesses, monitors, documents and reports patient progress using required documents.
• Collaborates with the interdisciplinary team to develop and manage the plan of care.
• Demonstrates competency in home health OT care and specialized skills.
• Prioritizes patient needs and uses sound judgment in care decisions.
• Assists with training and orienting team members.
• Completes timely, accurate documentation at the point of care.
