BAYADA Home Health Care is seeking a compassionate Occupational Therapist (OT) to join our Ocean County Visits team in New Jersey. You will provide one-on-one, in-home care to adult and geriatric clients, helping them stay safe, independent, and comfortable in their own homes.

New graduates are strongly encouraged to apply. The role offers flexible schedules, autonomy, and strong clinical support within a multidisciplinary team.

Formación

• Current New Jersey Occupational Therapist license or license-eligible.
• Graduate of an accredited Occupational Therapy program.
• New graduates strongly encouraged to apply.
• Prior home health experience preferred but not required.
• Strong clinical, organizational, communication, and problem-solving skills.
• Ability to work independently and effectively manage time.
• Experience with EMR documentation preferred.

Responsabilidades

• Provide skilled home visits within designated territories.
• Perform comprehensive evaluations of functional abilities (ADLs, IADLs, mobility, cognition, communication, home safety).
• Develop individualized treatment plans to improve functional outcomes and independence.
• Assess home environments and recommend adaptive equipment and modifications.
• Collaborate with nurses, therapists, physicians, and other healthcare professionals to deliver patient-centered care.
• Educate patients, families, and caregivers on therapeutic interventions and safety strategies.
• Accurately document assessments, treatments, and patient progress in EMR.
• Participate in interdisciplinary care planning to achieve optimal patient outcomes.
