Job summary

BAYADA Home Health Care is seeking an Occupational Therapist to join our Senior Living team, serving adults in Somerset, Middlesex, Monmouth, and Union Counties, NJ. This role focuses on maximizing independence while clients remain in their homes.

The OT will perform evaluations, develop plans, and collaborate with an interdisciplinary team. New Jersey OT licensure and at least one year of experience are required; home health experience is a plus. Competitive benefits and sign-on bonus apply.

Responsibilities

• Perform comprehensive OT evaluations and develop treatment plans.

• Deliver evidence-based interventions to promote independence.
• Collaborate with home health clinicians for transitions of care.
• Educate clients and families on plans, exercises, and safety.
• Complete documentation and discharge planning in the EMR.

Full-Time or Per Diem | Somerset, Middlesex, Monmouth & Union Counties, NJ

BAYADA Home Health Care is seeking a compassionate and experienced Occupational Therapist (OT) to join our Senior Living team. This position provides occupational therapy services to adult and geriatric clients in senior living communities throughout Somerset, Middlesex, Monmouth, and Union Counties, NJ.

As a BAYADA Occupational Therapist, you'll work as part of a collaborative interdisciplinary team dedicated to helping clients maximize their independence, safety, and quality of life while remaining in the place they call home.

Responsibilities

• Perform comprehensive occupational therapy evaluations and develop individualized treatment plans.
• Provide outpatient therapy services within senior living communities and group home settings serving adults with intellectual disabilities.
• Deliver evidence-based interventions that promote functional independence and improve quality of life.
• Collaborate with home health clinicians to ensure smooth transitions of care and achieve client-centered goals.
• Educate clients, families, and caregivers on treatment plans, home exercise programs, adaptive strategies, and safety recommendations.
• Complete timely documentation, treatment notes, and discharge planning using BAYADA's electronic medical record (EMR) system.

Qualifications

• Current New Jersey Occupational Therapist (OT) license.
• Minimum of one year of recent Occupational Therapist experience.
• Home health experience preferred, but not required.
• Graduate of an accredited Occupational Therapy program recognized by the AOTA and AMA.
• Excellent communication, organization, and time management skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Experience with electronic medical records (EMR) preferred.

Additional Information:

Base Pay, depending upon qualifications, status, and subject to negotiation:

$70.00 - $75.00 per point for full time status ($109,200-$117,000 annual earning potential)

$75.00 - $80.00 for per diem status

$10,000 Sign-On Bonus for Full-Time OTs
