Qualifications

• Licensed OT in the practicing state or eligible for licensure
• Graduate of an accredited Occupational Therapy program
• Eligible for NBCOT national registration
• Minimum 1 year of acute care experience
• Home health experience preferred (2 years)
• Access to reliable transportation

Responsibilities

• Provide prescribed occupational therapy in patients' homes
• Assess, plan, implement, and reassess therapy goals
• Collaborate with physicians and care team to promote independence
• Document progress and adjust plan of care every 30 days
