Freedom at Home in Southeast Michigan is seeking a compassionate Occupational Therapist to join our growing home health team. We offer excellent pay, flexible compensation options (salary or per-visit), paid mileage, and a sign-on bonus, with full-time opportunities.

As part of our clinician-first culture, you’ll evaluate patients in their homes, develop individualized plans, and collaborate with nurses and physicians to improve independence and safety.

Qualifikationen

• Graduate of an accredited Occupational Therapy program.
• Current Michigan Occupational Therapist license.
• Current CPR certification.
• Minimum of 2 years of Occupational Therapy experience preferred.
• Home Health experience is a plus but not required.
• Valid driver's license, reliable transportation, and auto insurance.

Aufgaben

• Evaluate patients in their homes and develop individualized treatment plans.
• Provide skilled occupational therapy services to improve independence and quality of life.
• Assess home safety and recommend adaptive equipment or modifications.
• Educate patients and caregivers on therapeutic exercises, self-care techniques, and safe use of assistive devices.
• Complete OASIS assessments and clinical documentation accurately and on time.
• Collaborate with physicians, nurses, and other healthcare professionals to achieve the best patient outcomes.
• Monitor patient progress and adjust treatment plans as appropriate.
• Promote safe, compassionate, high-quality patient care.
