Job summary

Boost Home Healthcare – Southeast Michigan is seeking a dedicated Physical Therapist to provide physician-prescribed PT services in patients' homes. You will assess and treat individuals to reduce disability and improve functional independence, coordinating with the care team to support recovery.

The role requires state licensure, a PT degree, and preferred home health experience, with familiarity in Medicare OASIS documentation.

Qualifications

• Degree in physical therapy from an accredited program (ABET/CAPTE-accredited)
• Licensed to practice physical therapy in the practicing state
• 2 years of home health experience preferred
• familiarity with Medicare OASIS documentation

Responsibilities

• Provide physician-prescribed physical therapy.
• Improve or minimize residual physical disabilities.
• Return the patient to optimum functional level.
• Collaborate with other home care personnel on patient care planning.
• Direct and supervise home care staff as required.
• Conduct initial assessment, plan of care, and 30‑day reassessments.
