Job summary

EvergreenHealth is seeking an Occupational Therapist to provide skilled home health services in the Seattle area. You will assess, plan, and implement therapy for patients in their homes, collaborating with families and the care team to achieve functional goals.

The role requires WA OT licensure, recent clinical experience, a current WA driver’s license with insurability, and a valid BLS certification. Training, flexible schedules, and strong patient outcomes are emphasized.

Qualifications

• Current OT licensure to practice Occupational Therapy in Washington State.
• Recent clinical experience in medical/surgical, rehab, or home health.
• Current Washington State Driver’s license and proof of insurability.
• Current Healthcare Provider BLS Certified by date of hire.
• Reliable transportation.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients in the patient’s home.
• Regularly evaluate and re-evaluate patient status.
• Initiate, develop, implement and revise the plan of care (POC).
• Address all problems identified in the POC.
• Educate patient/family/caregiver about condition and needs.
• Communicate with other disciplines involved in the case as warranted.
• Report signs/symptoms to physician and case manager as needed.
