Job summary

EvergreenHealth seeks an Occupational Therapist to provide home health services in Washington. The OT will assess, plan, implement and evaluate patient care in collaboration with the physician and interdisciplinary team.

Responsibilities include coordinating care, educating patients and families, and ensuring clinical outcomes meet industry benchmarks. A WA OT license and BLS are required, with a valid driver’s license and reliable transportation.

Qualifications

• Current OT licensure to practice Occupational Therapy in Washington State.
• Recent applicable clinical experience in a medical/surgical unit, rehab, or home health setting.
• Current Washington State Driver’s license and proof of insurability
• Current Healthcare Provider BLS certification by date of hire
• Reliable transportation

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients in the home, including instruction for patients and caregivers.
• Regularly evaluate and re-evaluate patient status.
• Initiate, develop and revise the patient’s plan of care (POC).
• Address all problems identified in the POC.
• Provide educational information to patient/family/caregiver.
• Communicate with other disciplines involved in the case.
• Report signs/symptoms signaling change in patient condition to physician/case manager.
• Reassess the patient within 48 hours following a change or discharge as ordered by physician.
