Job summary

An innovative home health provider is seeking an Occupational Therapist to provide skilled care in patients’ homes. Responsibilities include assessing and creating care plans while coordinating with interdisciplinary teams to achieve optimal patient outcomes. Qualifications

• Current OT licensure to practice Occupational Therapy in Washington State.
• Recent applicable clinical experience in a medical/surgical unit, rehab, or home health setting.
• Current Healthcare Provider BLS certification by date of hire.

Responsibilities

• Assessing, planning, implementing, evaluating and providing skilled care to patients requiring occupational therapy services.
• Coordinate and oversee skilled intermittent care to patients.
• Initiate, develop, implement and revise the patient’s plan of care.
