Job summary

EvergreenHealth seeks an Occupational Therapist to provide skilled home health care in the Seattle area. You will assess, plan, implement, and evaluate care with an interdisciplinary team, ensuring treatment aligns with physician orders and patient goals.

You will manage the plan of care, educate families, collaborate with nurses and therapists, and maintain timely reassessments and documentation. WA licensure and BLS are required.

Qualifications

• Current OT licensure to practice Occupational Therapy in Washington State.
• Recent applicable clinical experience in home health, medical/surgical, or rehab.
• Current Washington State Driver’s license and proof of insurability.
• Current Healthcare Provider BLS certification by date of hire.
• Reliable transportation.

Responsibilities

• Provide, coordinate and oversee skilled intermittent OT care in the patient\'s home.
• Regularly evaluate and re-evaluate patient status.
• Initiate, develop, implement and revise the patient\'s plan of care (POC).
• Address all problems identified in the POC.
• Educate patient/family/caregiver regarding knowledge deficits or needs.
• Communicate with other disciplines involved in the case as warranted.
• Report signs of change to physician/case manager as needed.
• Reassess the patient within 48 hours after condition changes or discharge.
