Job summary

Day Kimball Health is seeking a Per Visit Occupational Therapist to join our Homecare department in Thompson, Connecticut. The OT provides skilled services in patients' homes, evaluating function and developing plans to maximize independence and safety, while collaborating with a team to reduce hospitalizations.

The role emphasizes education for patients and families, coordination with physicians, and thorough documentation. A valid CT OT license and travel across the service area are required.

Qualifications

• Graduate of an accredited OT program with a Bachelor’s, Master’s, or Doctoral degree in Occupational Therapy.
• Current Connecticut Occupational Therapist license.
• Current BLS/CPR certification.
• Valid Connecticut driver’s license.
• Reliable transportation and proof of automobile insurance.
• Ability to travel throughout the agency service area.

Responsibilities

• Performs comprehensive occupational therapy evaluations in the patient’s home environment.
• Develops and implements individualized treatment plans based on physician orders and patient goals.
• Provides skilled OT interventions to improve functional independence, safety, and quality of life.
• Educates patients, caregivers, and family members regarding treatment plans and home exercise programs.
• Collaborates with physicians and interdisciplinary team to coordinate patient care.
• Maintains complete, accurate, timely clinical documentation per agency/regulatory requirements.
• Evaluates patient progress and revises treatment goals as needed.
