Job summary

Day Kimball Health in Thompson, CT is seeking a Per Visit Physical Therapist to join our Homecare team. You will administer prescribed physical therapy programs in patients' homes, aiming to reduce disability and maximize functional potential under supervision of the HomeCare Rehabilitation Supervisor.

Ideal candidates have home health, orthopedic, or fast-paced clinical experience, with independence, adaptability, and patient-centered care.

Qualifications

• B.S. Degree and graduate from an accredited school of physical therapy.
• Connecticut license or license eligible required.
• CPR Certification required.
• Minimum of one year experience preferred.
• Valid Connecticut Driver’s License and proof of current insurance coverage.

Responsibilities

• Evaluates and treats patients as prescribed by referring physician; records treatment and progress.
• Provides supervision of the Physical Therapist Assistant related to care plan implementation.
• Orients, instructs, and directs other Health Care Team members as needed.
