Job summary

Day Kimball Health in Thompson, CT is seeking a Per Visit Speech Language Pathologist to join our Homecare team. You will provide prescribed speech therapy services in patients' homes, manage your caseload, and collaborate with a Rehabilitation Supervisor.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC in Speech-Language Pathology required.
• Connecticut license or CT license eligible required.
• CPR Certification required.
• Minimum of one year experience preferred.

Responsibilities

• Evaluates and treats patients as prescribed by referring physician.
• Records treatment, response, progress and discharge information in patient’s chart.
• Educates patient, family and significant others regarding condition, treatment, home program and Rehabilitation Services.
• Routinely confers with physician and other practitioners regarding progress, regression or change of plan.
• Orients, instructs, and directs other Health Care Team members as needed.
