Job summary

Addus HomeCare, Inc. is seeking a licensed Occupational Therapist to deliver in-home care in the Middletown area under the plan of care, collaborating with family and the care team.

You will assess clients, educate on therapeutic self-care, design adaptive devices, document progress, and participate in care conferences. The role offers a flexible schedule, per-visit compensation, mileage reimbursement, and a 401(k) match.

Qualifications

• Graduate of an accredited occupational therapy curriculum.
• Certified as an occupational therapist by AOTA.
• Current Occupational Therapist license in the practicing state.
• Minimum one year of work experience as an Occupational Therapist.
• Must have a valid Driver’s license and CPR/First Aid certification.

Responsibilities

• Guide and instruct the client in prescribing therapeutic self-care and activities to improve independence.
• Participate in development of the plan of care.
• Prepare a diagnostic evaluation report and make recommendations.
• Prepare clinical and progress notes.
• Advise, consult and teach treatment procedures to clients, family and staff.
• Establish household management routines.
• Design/order adaptive devices or splints to promote functioning.
• Instruct clients and families in activities of daily living.
• Attend care conferences as needed.
• Maintain confidentiality and adhere to policies and HIPAA.
