Job summary

EvergreenHealth is seeking a licensed physical therapist to provide skilled home health services in the Bellevue, WA area. You will assess, plan, implement and evaluate care in the patient’s home, coordinating with families and an interdisciplinary team to achieve optimal outcomes.

Responsibilities include educating patients and caregivers, reevaluating progress, and updating plans of care. Requires WA PT license, recent home health or hospital rehab experience, and BLS certification.

Qualifications

• Current PT licensure to practice Physical Therapy in Washington State.
• Recent clinical experience in medical/surgical unit, rehab, or home health.
• Current Washington State Driver’s license and proof of insurability.
• Current Healthcare Provider BLS certification by date of hire.

Responsibilities

• Assess, plan, implement, evaluate home health physical therapy as ordered by physician.
• Provide skilled intermittent care and educate patients/caregivers to promote functional status.
• Regularly evaluate and re-evaluate patient status and adjust the POC.
• Initiate, develop and revise patient plans of care; address problems in the POC.
• Communicate with other disciplines and report changes to physician/case manager.
• Reassess within 48 hours of condition changes or discharge as ordered.

Education

Washington State PT licensure
