Job summary

Peace and Harmony Homecare LLC in Framingham, MA is seeking a licensed Occupational Therapist to provide home-based OT services under RN supervision. You will evaluate patients, develop and implement plans of care, and educate patients and families in activities of daily living.

Requirements include MA OT licensure, ACOTE-accredited OT education, NBCOT certification, and 1+ year home care experience. Travel to patient homes and collaboration with care teams are essential.

Qualifications

• Graduated from ACOTE-accredited OT program and eligible for NBCOT certification.
• Licensed OT in MA with current licensure.
• Minimum of 1 year OT experience, preferably in home care.
• Criminal check completed and CPR certification current.

Responsibilities

• Receive intake referrals and prepare OT paperwork for visits.
• Conduct home safety checks and environmental assessments.
• Perform OT evaluation and develop a Plan of Care for physician approval.
• Assess ADL, functional status, and progress to adjust therapy plan.
• Communicate with patient/family about POC progression and goals.
• Collaborate with RN case manager and other care team members.
• Complete documentation weekly and coordinate re-authorization if needed.
• Supervise home health aides as applicable and ensure compliance.
