Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Provides service within the scope of practice as defined by the state laws governing Occupational Therapy, per plan of care.
• Evaluates functional status and CMS-required documentation; consults with physician on plan of care.
• Observes, records, and reports patient response to treatment and changes in condition.
• Conducts patient assessments evaluating functional level using diagnostic and prognostic tests.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition.
