Job summary

Northern Arizona Home Health, part of the LHC Group and Optum family, seeks an Occupational Therapist to assess and address functional status, daily living activities, and adaptive equipment needs in home health settings.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Valid driver’s license and access to a dependable vehicle or reliable transportation.
• Preferred CPR certification or ability to complete within 90 days of hire; strong communication and organizational skills.

Responsibilities

• Provides service within the scope of practice as defined by state OT laws, coordinating with the health care team.
• Evaluates functional status and CMS documentation needs; consults with the physician on the care plan.
• Observes, records, and reports patient responses and changes to condition to the nurse supervisor and physician.
• Conducts patient assessments evaluating function using diagnostic and prognostic functional ability tests.
• Function in home environments regardless of age, race, creed, or financial condition of the client.
