Qualifications

• Active Occupational Therapy licensure in the state of practice.
• Current CPR certification.
• Valid driver''s license and access to a dependable vehicle.
• Ability to work in home settings with diverse populations.

Responsibilities

• Provides OT services within the scope of practice defined by state laws, according to the plan of care.
• Evaluates patient functional status and CMS documentation; consults with the physician.
• Observes, records and reports patient responses to treatment and changes to condition.
• Conducts patient assessments of functional ability using standardized tests.
