Job summary

Advocate Health is seeking a Home Health Occupational Therapist to manage an assigned patient caseload in patients' homes across the University City and surrounding Charlotte area. You will implement therapy plans, provide caregiver instruction, and document progress and outcomes while coordinating with the multidisciplinary team.

The role offers standard benefits, a 40-hour workweek, and a sign-on bonus up to $15,000 for qualified applicants, plus a $5,000 referral bonus for internal teammates.

Responsibilities

• Manage assigned patient caseload in home care setting and document per plan of care and MD orders.
• Collect patient data from assessments, telephonic contacts, and EMR sources.
• Implement OT care programs, modalities, exercise programs, and patient education as prescribed.
• Develop and document a plan of care including discharge planning and resources.
