Qualifications

• Active California professional license (PT or OT).
• Independent contractor (1099) status.
• Ability to work in patients' homes and coordinate care.

Responsibilities

• Submit referrals and arrange sessions using the Suru app.
• Conduct Telehealth evaluations when appropriate.
• Provide in-home PT/OT visits with consistent scheduling.
• Submit visit notes and treatment plans.
• Coordinate with Suru for eligibility and billing.
