All Care Precision Orthopedic Clinic in Smithtown, NY, seeks a dedicated Orthopedic Physical Therapist for a full-time, home-care role. You will evaluate, treat, and guide patients in their recovery, delivering compassionate, one-on-one care in clients' homes.

You will design personalized plans using exercise, manual therapy, and modalities, document progress, educate families, and stay current through workshops and collaboration with the team.

Qualifications

• DPT from an accredited program is required.
• Valid New York state license to practice physical therapy.
• Strong written and verbal communication for patient interaction and documentation.
• Proficiency with word processing and basic computer systems for EHRs.
• Flexible and adaptable mindset to adjust treatment plans as patient needs change.

Responsibilities

• Conduct home-based evaluations of patients recovering from orthopedic injuries or surgeries.
• Design and implement personalized treatment plans with exercises, manual therapy, and modalities.
• Document progress and adjust goals after ongoing evaluations.
• Educate families and caregivers on exercises and assistive devices.
