Apex Rehabilitation is seeking a Home Care Physical Therapist to join a compassionate team delivering in-home care to clients in Long Island. You will help clients regain mobility, reduce pain, and improve overall well-being in the comfort of their homes.

Qualifications

• Degree in Physical Therapy from an accredited program.
• State licensure as a Physical Therapist.
• Strong clinical and interpersonal skills.
• Ability to adapt therapy plans to meet home care needs.
• Compassionate and patient-centered approach to care.

Responsibilities

• Conduct comprehensive assessments of clients' physical conditions in a home care setting.
• Develop personalized and goal-oriented physical therapy plans based on assessment findings.
• Administer therapeutic exercises, manual therapy, and other interventions to enhance clients' functional abilities.
• Collaborate with clients, their families, and other healthcare professionals to ensure coordinated care.
• Educate clients on proper techniques for home exercises and self-care.
• Maintain accurate and thorough documentation of assessments, interventions, and progress.
