Job summary

Caringhand Home Health Solutions, Inc. is seeking an experienced Occupational Therapist to help patients regain independence in their own homes. You will assess functional abilities, create personalized treatment plans, and coach patients in daily living activities to improve safety and autonomy.

This field-based, per diem role requires travel to patients' homes in California. Competitive compensation is provided per visit, with opportunities to convert to full-time based on performance.

Qualifications

• OT degree from ACOTE-accredited program.
• California OT license active.
• CPR certification (AHA) current.
• Valid driver’s license and reliable vehicle; willingness to travel.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services
