Job summary

Caringhand Home Health Solutions, Inc. is looking for an Occupational Therapist to help patients regain independence and improve daily living at home in California.

This per diem role offers a $130 per visit rate and requires travel to patient homes to assess needs and provide skilled care. Responsibilities include evaluating home environments, developing treatment plans, teaching daily living skills, coordinating with the care team, and supporting discharge planning as patients transition from

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the AMA and AOTA.
• Registered with the NBCOT/National Registration Examination of the AOTA.
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver’s license and ability to travel to patient homes.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
