Job summary

Caringhand Home Health Solutions, Inc is seeking an Occupational Therapist to provide home-based services, traveling to patients' homes to assess needs and support independence. This field-based, per diem role is not remote or telehealth; compensation is $130 per visit.

Responsibilities include evaluating functional abilities, developing treatment plans, training patients, coordinating with care teams, and educating families.

Qualifications

• Graduate of an occupational therapy curriculum accredited by AMA/AOTA.
• Registered with the National Registration Examination of the American Occupational Therapy Association.
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.

Responsibilities

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient's treatment approach.
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
