CenterWell Home Health in the United States seeks a compassionate Home Health Occupational Therapist to support patients at home. You will assess daily living and work-related skills, develop retraining plans, and collaborate with physicians and the care team to update the Plan of Care.

You will provide therapeutic treatment to restore strength and range of motion, document progress, and educate patients, families, and staff on rehabilitation techniques.

Qualifications

• Current and unrestricted OT licensure.
• CPR certification required.
• Valid driver's license and reliable transportation.

Responsabilités

• Assess and screen daily living and work-related skills; develop retraining programs with clear objectives.
• Administer and interpret functional tests to support physician evaluation.
• Collaborate with the care team to update the Plan of Care.
