Job summary

Angels of Mercy Homecare - Marion, a part of the LHC Group and a member of the Optum family, seeks an Occupational Therapist to assess and address patients’ functional needs in home health settings. In this role you will evaluate ADLs, coordinate with the care team, and develop plans of care to promote independence and safe daily living for clients in their homes.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other health care team members.
• Evaluates the patient's functional status and CMS documentation readiness; consults with physician in care plan development.
• Observes, records, and reports patient responses to treatment to the nurse supervisor and physician.
• Conducts patient assessments evaluating function using diagnostic and prognostic tests.
