Job summary

Angels of Mercy Homecare - Marion, a part of LHC Group, seeks an Occupational Therapist to assess and address patient needs in home health, including functional status, ADLs, and home safety. Relocation assistance is provided for external candidates.

The role requires state OT licensure and CPR certification, plus a valid driver’s license and reliable transportation. You will work with a diverse patient population in a supportive team environment and comprehensive benefits package.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy, in accordance with the plan of care and in coordination with other health care team members.
• Evaluates patient functional status and CMS documentation needs; consults with physician in care plan development.
• Observes, records, and reports patient response to treatment and changes to condition.
• Conducts patient assessments evaluating functional level using diagnostic and prognostic tests.
