Job summary

Angels of Mercy Homecare - Marion, part of the LHC Group and within the Optum family, seeks an Occupational Therapist for home health. You will assess functional status, daily living activities, and home safety to plan therapy interventions tailored to each patient.

You will work with physicians and nurses to implement care plans, document outcomes for CMS, and educate patients and families on adaptive equipment and activities. A compassionate, collaborative approach is essential.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Ability to function in home health settings and coordinate with health care team.

Responsibilities

• Assess and evaluate patient care needs related to functional status and ADLs.
• Develop and implement therapy plans of care in collaboration with physicians.
• Record patient progress and communicate changes to the team.
• Provide education and support to patients and caregivers.
