Job summary

Compassus in Wisconsin is seeking a Home Health Occupational Therapist to deliver one-on-one care in patients' homes, helping them live safely and independently.

You will assess home environments, adapt tasks, and educate families while maintaining a BS in OT, current state license, and a valid driver’s license. At least two years of acute care or rehab OT experience and CPR/TB/Hepatitis clearances are required.

Qualifications

• Bachelor of Science in Occupational Therapy required.
• Current State License as an Occupational Therapist.
• Valid Driver’s License.
• CPR certification, TB screen, and Hepatitis consent/declination.

Responsibilities

• Provide one-on-one OT services in patients' homes.
• Assess home environments and adapt tasks/equipment to improve independence.
• Educate patients and families to enhance daily living activities.
• Collaborate with other care team members to ensure consistent, quality care.
