Responsibilities

• Provides service within the scope of practice as defined by state laws, in accordance with the plan of care and in coordination with the health care team.
• Evaluates the patient's functional status and CMS-required documentation; consults with the physician to develop the plan of care.
• Observes, records, and reports changes in patient condition to the nurse supervisor and physician.
• Conducts patient assessments evaluating functional ability using diagnostic tests and prognostic tools.
• Functions effectively in diverse home settings and with patients of varied backgrounds.
