Job summary

Saint Mary’s Homecare, part of LHC Group and within the Optum family, seeks an Occupational Therapist in Home Health to assess patient care needs, including functional status, ADLs, and home adaptations as defined by medical conditions.

Primary responsibilities include delivering OT services per state scope, coordinating with the care team, documenting progress, and supporting patients in diverse home settings.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• Demonstrated ability to manage multiple tasks simultaneously and work independently.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing OT and plan of care.
• Evaluates functional status and collaborates with physician in developing the therapy plan of care.
• Observes, records, and reports patient response to treatment to nurse supervisor and physician.
• Conducts patient assessments evaluating function using diagnostic and prognostic tests.
• Function in home settings regardless of age, race, creed, color, sex, disability, or financial condition.
