Job summary

Concierge Home Care is seeking a Home Health Occupational Therapist to deliver 1:1 home-based care in the St. Cloud area (Osceola County, FL). This Full-Time, Pay-Per-Visit role offers autonomy to manage your own schedule and a path to higher earnings through productive patient volumes.

You will assess patients, develop individualized treatment plans, and collaborate with physicians. Requires active Florida OT license, CPR, valid driver’s license, and ability to supervise COTAs; home health

Qualifications

• Active, unencumbered Florida Occupational Therapist license.
• Valid driver’s license and reliable transportation.
• CPR certification.
• Ability to supervise COTAs.
• Home health experience preferred, but not required.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Perform home safety assessments and recommend modifications.
• Provide therapy to improve ADLs, mobility, and independence.
• Educate patients and caregivers on adaptive techniques and equipment.
• Collaborate with physicians and interdisciplinary teams.
• Supervise Certified Occupational Therapy Assistants (COTAs).
• Complete documentation using streamlined technology.

Your Role as a Home Health Occupational Therapist
