Job summary

PurposeCare in South Bend, Indiana, is seeking an Occupational Therapist to join our home health team. This paid per-visit role offers flexible scheduling, autonomy to make clinical decisions, and the opportunity to deliver high-quality care in patients' homes.

Requirements include an active OT license in Indiana, up to 1 year experience preferred in home health, reliable transportation, and ability to lift up to 50 lbs. Rates range from $66 to $75 per visit; we are an Equal Opportunity Employer.

Qualifications

• Current Occupational Therapist license in Indiana.
• 1 year of occupational therapist experience is preferred - home health and OASIS experience preferred.
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing.
• Current Driver's License, vehicle insurance, and/or access to dependable transportation.
• Ability to successfully complete a background check.

Responsibilities

• Provide skilled visits that address the full scope of a patient's needs in their home.
• Collaborate with patients and families to develop personalized care plans and adjust as needed.
• Document assessments, progress, and outcomes in the patient's charting system.
