Job summary

PurposeCare seeks a passionate Occupational Therapist to deliver skilled home health visits, designing patient-centered plans and empowering clients to regain independence in their own homes.

Qualifications

• Current Occupational Therapist license in the state of application.
• 1 year of occupational therapist experience preferred; home health and OASIS experience preferred.
• Ability to perform tasks involving physical activity, including lifting up to 50 lbs.
• Valid driver’s license or reliable transportation and vehicle insurance.
• Ability to pass a background check.

Responsibilities

• Provide skilled OT visits in patients' homes.
• Enable progress in daily living activities and rehabilitation at home.
• Coordinate care with patients and families to support recovery.
