UnitedHealth Group is seeking an Occupational Therapist to join our Home Health team. You will assess patient needs related to functional status, daily living activities, fine motor skills, home safety, and adaptive equipment as defined by medical conditions.

Responsibilities include evaluating functional status per CMS documentation, coordinating with physicians to develop plans of care, and documenting patient progress. Must travel to patients' homes as required.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsabilités

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other health care team.
• Evaluates the patient's functional status and CMS documentation needs; consults with physician in plan of care.
• Observes, records, and reports patient responses and changes to condition to nurse supervisor and physician.
• Conducts patient assessments evaluating function using diagnostic and prognostic tests.
• Function in home settings regardless of age, race, or disability.
