Job summary

UnitedHealth Group is seeking an Occupational Therapist in Home Health to assess and address patient needs related to functional status, ADLs, and home adaptations. You will coordinate with the care team and physician to develop and monitor therapy plans, ensuring evidence-based, patient-centered care in the home setting.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance or access to dependable transportation.
• Ability to function in diverse home settings regardless of client background.

Responsibilities

• Provides service within scope of practice as defined by state laws governing Occupational Therapy, coordinating with the plan of care.
• Evaluates patient functional status and CMS-required documentation, in collaboration with the physician.
• Observes, records, and reports patient responses to treatment to the nurse supervisor and physician.
• Conducts patient assessments evaluating functional level using standard tests.
