Qualifikationen

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance or access to reliable transportation.
• CPR certification preferred or within 90 days of hire.
• Ability to manage multiple tasks and work independently.

Aufgaben

• Assess and evaluate patient care needs related to functional status and activities of daily living.
• Perform home assessments and recommend adaptive equipment as defined by the plan of care.
• Coordinate with physicians and health care team to develop therapy plan of care.
• Document patient progress and report changes to supervising nurse and physician.
• Provide services in accordance with state OT practice laws and CMS documentation requirements.
