Elara Caring is seeking a Home Health Occupational Therapist to join our team in Massachusetts. The role focuses on delivering high-quality home-based therapy to patients in their own homes, with flexible hours and a strong emphasis on safety and independence.

The selected OT will conduct patient assessments, develop and implement plans of care, coordinate with a multidisciplinary team, and educate patients and families on activities of daily living. Travel within the branch area is required.

Competenze

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one year of clinical care experience.
• Able and willing to travel up to 50%.

Mansioni

• Evaluate OT patient needs and determine rehabilitation goals.
• Develop and implement the Plan of Care to ensure quality care.
• Coordinate with the care team for discharge planning and follow-up.
• Instruct patients and families in activities of daily living and home program.
