Job summary

Elara Caring in Needham, Massachusetts is seeking a Home Health Occupational Therapist to provide therapy services in home settings. The OT will evaluate patients, develop and implement Plan of Care, and educate patients and families on daily living activities and home safety.

Requires graduation from an accredited OT program, an active OT license in MA, and at least 1 year of clinical experience. Travel up to 50% is required with a valid driver’s license.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one year of experience in a clinical care setting.
• Able and willing to travel 50%.

Responsibilities

• Evaluates occupational therapy patients to determine rehabilitation needs and plans care.
• Implements and updates plan of care to ensure quality and continuity.
• Communicates findings to supervisor, physician and care team.
• Instructs patients and families in activities of daily living and home programs.
• Supervises clinical staff to ensure therapy care plan adherence.
• Maintains documentation and adheres to standards and regulations.
