Job summary

Premier Home Health Providers Inc. is seeking a per diem Occupational Therapist to evaluate patients in home settings, develop treatment plans, and educate families on daily living activities. The role emphasizes safe home navigation and discharge planning.

Requirements include California OT licensure, NBCOT registration, CPR, and at least 2 years of clinical OT experience. A valid driver’s license and willingness to travel are essential.

Qualifications

• Graduate of an occupational therapy curriculum accredited by the AMA/AAOTA.
• Registered with the NBCOT (National Registration Examination).
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current AHA CPR certification.
• Valid driver’s license with insured vehicle and ability to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment and devices.
• Maintain accurate clinical records and report changes in patient condition.
• Coordinate with the care team for seamless, patient-centered care.
• Educate family members and caregivers on the treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.
