The role emphasizes collaboration with a multidisciplinary team, education for patients and families, and documentation in electronic health records. Flexibility to travel between patients' homes is required.

Qualifications

• Graduate of an accredited occupational therapy program recognized by state and national bodies.
• Current, unrestricted New Hampshire OT license.
• Knowledge of OT principles, assessment, treatment interventions, adaptive equipment, and home safety.
• Ability to work independently and manage an assigned caseload.
• Strong communication and teamwork with patients, families, and the care team.

Responsibilities

• Conduct comprehensive OT evaluations and ongoing assessments of patients' needs.
• Develop individualized, measurable plans of care based on assessments and goals.
• Provide skilled OT interventions to improve or maintain function and independence.
• Address ADLs, IADLs, mobility, transfers, safety, and home adaptations.
• Evaluate progress and modify plans based on changes and barriers.
• Educate patients and caregivers on activities, equipment use, and safety.
