The OT will evaluate patients in their homes, set goals and develop a plan to help them achieve independence and improve quality of life, while preventing potential health-related problems. Qualifications

• Bachelor's degree in OT from an accredited program.
• Active OT license to practice in the state.
• CPR/BLS certification required.
• 1 year OT clinical experience.
• Current driver's license.

Responsibilities

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs.
• Develop and implement personalized treatment plans including exercises and adaptive devices.
• Educate patients and families on home safety and daily activities.
• Maintain accurate records of patient progress and treatment plans for compliance.
• Coordinate care with physicians, nurses, therapists and home health team.
