Job summary

Residential Home Health and Residential Hospice seeks a Home Health Occupational Therapist to provide skilled OT services in patients’ homes in Marion, IL. Eligible for a $20,000 total bonus potential, including a $10,000 first-year success bonus and a $10,000 sign-on bonus paid within 18 months.

You will evaluate ADLs and home safety, develop individualized plans, educate patients and caregivers, and collaborate with an interdisciplinary team.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life
• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

OT Qualifications

• Active Occupational Therapist, OT license in the state you work
• One year of experience as an Occupational Therapist and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
