Job summary

Residential Home Health and Hospice is hiring an Occupational Therapist to provide skilled home health care in the Lancaster, PA area. The role includes driving to patient homes, delivering focused ADL and safety evaluations, and building individualized treatment plans to promote independence.

You will educate patients and caregivers on adaptive techniques and safety strategies, while collaborating with a supportive interdisciplinary team and maintaining compliant EMR documentation.

Qualifications

• Active Occupational Therapist (OT) license in the state you work.
• One year of experience as an OT delivering quality patient care.
• Valid drivers license and ability to travel ~20% of the workday to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in home health, collaborating with an interdisciplinary team.
• Perform comprehensive ADL, upper extremity, and home safety evaluations and develop individualized treatment plans.
• Educate patients and caregivers on adaptive techniques, energy conservation, and home safety strategies.
• Think critically, document thoroughly in EMR, and work independently while staying connected with the team.
• Travel to patient homes within the assigned territory.

What You'll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life
• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

OT Qualifications

• Active Occupational Therapist, OT license in the state you work
• One year of experience as an Occupational Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
