Residential Home Health and Hospice in Milford, PA is seeking an Occupational Therapist to provide skilled home health care to patients in their homes. • Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform comprehensive patient evaluations focused on ADLs, upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life.
• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Travel to patient homes within assigned territory.

What You'll Do:

• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone

• Travel to patient homes within assigned territory
